Manual Test Orders
Submit all laboratory test requests in writing on a Cleveland Clinic Laboratories Test Request Form. Verbal requests are not accepted. Cleveland Clinic Laboratories provides preprinted, client specific test request forms for use in ordering laboratory testing.
Provide complete patient information on the form using block printing for accuracy. Always include the patient's medical record or Social Security number on the form.
